If you need to update your company information, please follow the relevant section below and download the corresponding form.
1. Change of Contact / Billing Address / Machine Location
Please complete the Change of Company Information Request Form with an authorized signature and company chop
2. Change of Company Name
- Completed the Change of Company Information Request Form with an authorized signature and company chop
- Certificate of Change of Name issued by the Companies Registry
- Certified copy of the new company Business Registration Certificate
3. Change of Machine Ownership
- Completed the Change of Company Information Request Form with an authorized signature and company chop
- Written notification issued by the old company or fill in Change of Company Information Request Form completed by both the old and new companies, with authorized signatures and company chops from both parties.
- Certified copy of the new company Business Registration Certificate
